Insurance Information

We currently participate with most insurance plans.

Please contact our office if you need assistance establishing whether or not we are a participating provider with your insurance plan.

As always, patients are strongly encouraged to contact their specific insurance provider prior to their visit to determine whether you have allergy benefits under your plan. In addition, it is your responsibility to be aware of any potential out-of-pocket costs prior to your visit. Out-of-pocket costs may exist under your plan even if allergy services are considered covered benefits.

Billing Policies

Payment is expected at the time of service, unless other arrangements have been made. For your convenience, we accept checks, cash, Visa, MasterCard, and Discover.

Office visit co-pays are collected at the time of registration.

If we participate with your insurance carrier(s), we will bill them on your behalf. In order to bill your insurance properly, your insurance card(s) and driver's license must be presented at registration.
